Cancellation Request

To protect our customers and avoid any problems, we cannot accept telephone cancellations.Cancellation notices must be submitted in ONE of the following written methods and must include the following information: your name, userid, password (for account verification) & current return e-mail address. Keep in mind that cancellations submitted through the form on this page are dated the day they are sent from. Acceptable methods of written cancellation requests are:
